Partners Behavioral Health Management is a managed care organization that oversees federal, state, and county funds for individuals who need services for mental health, substance use disorders, and/or intellectual/developmental disabilities who have Medicaid or are uninsured or under-insured. Southern Family Medicine offers Medication Assisted Treatment (MAT) for opioid use disorder. Their treatment model allows for weekly, bi-weekly, or monthly appointments. The Children’s Developmental Services Agency (CDSA) is a regional early intervention center for infants and toddlers with developmental disabilities or delays. The purpose of early intervention is to help families promote their child’s development and provide needed information and support. Compassion Care Center is a medical clinic offering limited ultrasounds, pregnancy tests, prenatal/parenting classes, STD testing/treatment and post abortion support groups. Hands of Hope Medical Clinic

Hands of Hope Medical Clinic provides free primary medical care to Yadkin County residents between the ages of 18-64 who are uninsured, unable to afford healthcare from any other source, and not receiving Medicaid or Medicare. The walk-in clinic opens on Tuesday evenings at 5:00 p.m. Patients are seen on a first come, first served basis. Appointments cannot be made. High Country Community Health Pediatrics

High Country Community Health is a Patient Centered Medical Home that provides primary medical care services to pediatric patients, regardless of insurance status or ability to pay. HCCH also offers behavioral health services, including Mental Health Counseling and access to a Patient Resource Specialist. Insight Human Services – Prevention

Insight Human Services provides substance misuse prevention services to 13 counties in North Carolina. Trained Program Specialists develop meaningful relationships in the communities they serve to tailor a useful array of services geared toward population-based changes. Insight Human Services also provides educational opportunities in schools, childcare facilities, after school programs and other youth serving initiatives.


Novant Health First Steps

Novant Health First Steps provides postpartum support to new moms and families who deliver a baby at Novant Health Forsyth Medical Center.
Registered nurses will follow-up with families by phone between 1 and 3 weeks after a baby is born to check on how the family is adjusting, check on mom and babies’ health, and to provide education. The nurse will also make referrals for families to various community agencies if needed. A scheduler for the program will come to your hospital room after delivery to schedule your postpartum phone call.

The Care Management Program CMHRP and CMARC (formerly OBCM and CC4C)

The Care Management Program formerly OBCM promotes healthy mothers and healthy babies and provides care management services for high-risk women during pregnancy through 2 months after delivery. Formerly CC4C Care Coordination for Children is also housed under Care Management as CMARC. CMARC provides referral and coordination of services for children birth to age 5 who are at risk for developmental delay, long-term stressful situations, and special health care needs.

NETworX Yadkin

Our mission focuses on supporting the whole child, including: health; early literacy and education; and child development. An Early Childhood Resource Center that includes a lending library and resource room is available for early childhood educators and parents.

The Yadkin County Human Services Agency is comprised of the Women, Infant, and Children (WIC) Division, the Medical Clinic (*see Health and Developmental Services page), and the Social Services Division. The Social Services Division includes such programs as Food & Nutrition, Medicaid, Child Support, Child Care Subsidy, Emergency Intake, Work First, Adult Services, and Child Welfare Services. The agency also houses Environmental Health (213 East Elm Street; Yadkinville) which permits both old and new septic systems and inspects all food service and lodging facilities, as well as child care facilities, nursing homes, and public swimming pools in the county. A Child Care Health Consultant is also employed through the agency.

Rebuilding Lives Ministry

With a foundation of hope teens, adults, and families are empowered through spiritual counseling, engaged in educational workshops, and made aware of healthy behaviors that assist in overcoming addiction, allowing for personal motivation and strength to lead productive lives. Help is available in confronting traumatic events that have led to addiction and/or mental health concerns. Support is offered by a North Carolina Certified Peer Support Specialist (NC-CPSS) and minister to provide guidance and counsel. One-on-one conversations and group classes are available.


Solus Christus

Solus Christus is a Christian safe house for women experiencing homelessness, trauma, and addiction. They provide a safe place to stay while they help women get into long-term treatment.
